Real Care Adult Day Healthcare Center offers a safe, easy, and nurturing daytime program providing care and support for elder adults and disabled individuals. Our staff provides a family-like environment for all individuals to feel safe and to feel at home. We offer breakfast and fresh lunches, as well as healthy afternoon snacks and amenities. Our staff members pay specific attention and provide continuous supervision to each and every individual throughout each and every day for the utmost care.

ELIGIBILITY CRITERIA
-
Ability to be involved in therapy and/or program activities for a minimum of four (4) hours on scheduled days of attendance.

Existence of a medical condition that requires treatment or rehabilitation.

Need for ongoing nursing care or supervision.
-
Limited ability to perform personal needs and limited potential for self-care training without the support of ADHC support.

Limitation in movement, with or without assistive devices or may have need for training in use of devices.

With the support of the ADHC services, the ability of the person or family to cope adequately with problems associated with the disability.

Need for psychosocial environment involving peer group interaction.
-
Existence of mental impairments which handicap social functioning but do not require psychiatric institutional care.
